10 days later / 10 jours après

Je suis impatiente! Hier soir je me suis couchée sur le dos pendant que je regardais un film, à peine une heure comme cela, mais ce matin, j'avais très mal au niveau de la plaie. Bizarre car je ne le sentais pas hier soir. Je pensais que c'était une étape franchie mais non.

Je suis fatiguée après le moindre effort. Je dois rester allongée puis me lever pour une très courte période. Impossible d'envisager une sortie même à pied. Je suis trop faible. En plus je n'ai pas d'appétit.

J'attends les infirmières pour refaire le pansement et peut être enlever les points. Mercredi elle a dit qu'ils n'étaient pas prêts à enlever. Le chirurgien avait marqué jeudi mais on est vendredi aujourd'hui puisque les infirmières ne viennent que tous les deux jours. Je me sentirai mieux après que l'on me les enlève. Je l'espère en tout cas.

I am impatient! Last night I laid on my back whilst watching a film. I hardly stayed an hour like that but this morning I had terrible pain around the site of the wound. It's strange because I didn't feel it last night. I thought that I had made progress to another stage of healing but it appears I was wrong.

I am exhausted after the slightest effort. I have to lie down most of the time and get up for a very short period, around half an hour at present. It is out of the question to think about going out even for a short walk. I am too weak. I have no appetite either.

I am waiting for the nurses to come to re-do the dressing and maybe take out the stitches. On Wednesday the nurse said the stitches weren't ready. The surgeon said Thursday but it's now Friday as the nurses come only every two days. I think it might feel better once the stitches are out. Hope so anyway.

The trip home

7 hours but lying down in an ambulance. Got a bit of cramp but managed fairly well. Kept on either side but have avoided lying on my back as the doctor said it is bad for healing the wound. Changed position every 15 minutes.

Sept heures couchée en ambulance. Ai eu des crampes mais me suis débrouillée pas mal. Je suis restée sur le côté. Le médecin m'avait dit qu'être couchée sur le dos empêche la cicatrisation. J'ai changé de côté toutes les 15 minutes.

    

Operation : article 2

Je suis a J+6 de l'opération 

Ils ont enlevé le redon et les perfusions jeudi à J+4. C'est plus facile sans tous ces tuyaux! Je ne ressens pas trop de changement dans mon état depuis mon dernier article. Bien sûr il y a la fatigue inhabituelle. La bouée n'est plus une option alors c'est soit allongée ou debout. La bouée appuyerait sur la cicatrice. Les douleurs (plutôt le gène, que de vraies douleurs invalidantes) sont calmées par Doliprane mais je n'en prends pas les 8 cachets autorisés dans une journée. Je sens des tiraillements et des douleurs si je reste allongée trop longtemps. C'est la position qui ne convient pas. Il faut éviter d'aller sur le dos car la cicatrice devient comprimée. Les journées sont longues à l'hôpital mais vous êtes obligé de vous reposer ici. Ça c'est la clé de la récupération, repos, repos. Je me suis obligée de ne pas sortir de la chambre. Je reste dedans, je circule, je me lave (pas de douche encore), je lis, j'écoute des livres audio, de la musique. Je ne me penche pas. En fait j'ai perfectionné une technique pour ramasser des choses avec le pied! J'arrive à lever le pied jusqu'au genou pour rattraper ce que j'ai laissé tomber. Mon corps me dit que je ne dois pas me pencher. C'est facile pour moi, seule. La dame à côté a subi la même intervention mais elle a un enfant de 18 mois...

Je sors de l'hôpital demain à 13h. L e trajet en ambulance durera environ 7 heures. Je vous raconterai cela prochainement.

Pour ceux et celles que redoutent d'aller à la selle après cette opération, soyez tranquilles! Ça se passe très bien. Le plus important c'est la façon dont est faite le pansement car celui-ci protège le contact entre l'anus et la plaie. Professeur Doursounian a lui-même assisté au premier changement du pansement. Il m'a donné un protocole pour les infirmières qui me referont les pansements tous les deux jours chez moi. Je ferai retirer les points jeudi 28 novembre à J+10 de l'opération.

It is six days since the operation. They took the drips and the drain out after four days. Everything is much easier without all those tubes! I don't feel a great change in my condition since the last post. Of course there is a feeling of uncustomary tiredness. The donut is a no-no, so it is either laid down one your side or standing. The donut would press on the wound. The site has shifted to a higher spot where he has opened the spine. You can't put your bottom in the donut for relief as the operation is an inch above the anus in your lower back. The pain (or the uncomfortableness, rather than real debilitating pain) is relieved by Paracetamol but I am not taking the 8 per day allowed. I get a pulling sensation and pain if I stay lying down for too long, but not when sleeping. I think it is because I am trying to read or watch a screen so my head is turned whereas my body is already on its side. I have to avoid lying or pressing on my back as it squashes into the wound.

Days are long in hospital but you are forced to rest here. That appears to be the key to recovery, rest, rest. I have forced myself not to leave the room. I stay in and move around, wash, not showered yet. I read, listen to audio books, work on my forthcoming novel (publication April 2014), listen to music.

I don't bend over. I have perfected a technique to pick things up that I drop with my feet. I get the item between my toes and lift up my foot to my knee, then I can get it, without pain or stretching. My body is telling me not to lean over. It is easy for me on my own. The lady in the room next door has had the same operation and has a child of 18 months...

I leave hospital tomorrow at 1pm. The journey by ambulance will take around 7 hours. I will tell you about that shortly. 

For those of you who are worried like I was about moving your bowels after the operation, don't be! It is fine. The most important is the way the dressing has been done because this acts as a barrier between the wound and the anus. Professor Doursounian himself was there for the first dressing change and instructed the nurse. He gave me a printed sheet to give to the local nurses to explain how to do the dressing. They will be coming every two days to do it. The stitches will come out on Thursday  28th, 10 days after the operation.

Operation

Je suis à J+4 de l'opération. Je me sens bien. Je n'ai pas le wifi donc vous aurez ce poste rétroactivement, une fois que je serai rentrée dans le monde! Je vous fais une chronologie des 5 derniers jours car je suis arrivée à l'hôpital dimanche soir.

DIMANCHE, la veille de l'opération  

Ma tension était à 17/9! Pas étonnant. J'avais la trouille! En arrivant, sur la table de la chambre j'ai retrouvé un lavement. J'étais sensée le faire moi-même! J'avais beau essayé mais finalement j'ai appelé l'infirmier pour terminer "l'opération". Puis on me demande de me doucher avec de la Betadine, le soir et le lendemain matin. 

LUNDI J1

A 7h pétantes on vient me donner deux cachets pour me calmer avant l'opération. Franchement entre 7h et 9h45 je n'ai pas de souvenir de ce qui s'est passé. J'ai juste entendu quelqu'un dire que les opérations étaient toutes retardées car ils ouvraient le nouveau bloc opératoire. J'étais la première patiente. On m'a mis un masque avec de l'oxygène et prochaine étape, je suis en réanimation et il est 13h. Je n'avais pas mal du tout. Et je n'ai toujours pas vraiment mal. Quant au bas de contention personne ne me les a mises. 

Je suis restée longtemps en réanimation. Je suis retournée à la chambre vers 15h. L'opération avait duré une heure et quart. 

J'avais deux perfusions, une d'antibiotiques, et une deuxième de calmants. Aussi un tuyau qui s'appelle un redon sortant de l'incision. Ce tuyau va dans une bouteille et collecte le sang et les fluides qui coulent a l'intérieur de la plaie. Pendant les 4 jours il a drainé 27 cl. (Une grande tasse)

17h On me présente la bassine et même avec les calmants je sens que cela me fait mal derrière. Évidemment. En plus je n'arrive pas a faire pipi. On parle de sonde.

19h on me donne deux verres d'eau plus une injection d'anti coagulant.

19h30 le professeur Doursounian arrive et voit la bassine. "Mais elle ne peut pas utiliser cela, amenez-la au WC!"dit-il. 

Réussite spectaculaire et immédiate! Heureusement qu'il est passé me voir! La première nuit je me suis levée à 4h avec de l'aide, car j'avais l'oxygène sur le visage, les deux perfusions et le redon. Pas trop de difficulté et presque pas de douleur.

It is 4 days since the operation. I feel well. I haven't got wifi so you will get these posts days after the actual operation once I get back to the technological world. I took notes right through so what you read is exactly what happened from the Sunday evening when I arrived. 

SUNDAY, day before operation

My blood pressure measured 170/90! Not surprising. I was terrified. When I walked into the bedroom, there on the table was am enema. I was supposed to give it to myself! I tried but in the end called a nurse who finished it off for me. Then I was asked to shower with Betadine (the yellow staining stuff) that evening and the following morning.

MONDAY DAY 1

At 7am precisely, a nurse came to give me two tablets to calm me down before the operation. Pre-meds. Honestly between 7am and 9.45am I have no idea what happened. I just heard someone saying that all the operations were late because they were inaugurating a new operating block. I was its first patient. They put an oxygen mask on my face and next thing I know I was in the recovery room and it was 1pm. I had no pain whatsoever. I still don't have any real pain. As for the support stockings nobody put them on me. 

I stayed in the recovery room for a long time. I was wheeled back to the room around 3pm. The operation had lasted an hour and a quarter.

There were two drips, one with antibiotics and the other with painkillers and fluids. There was a drain too from the wound. The pipe from the interior of the wound drains blood and fluids into a bottle. Over four days it drained 27 centilitres.(a large mug)

5pm They came in with a bed pan. Even with the pain killers I could feel it touching the wound and it hurt. Added to that I couldn't go. They started talking about a catheter.

7pm Two glasses of water and an injection of anti-coagulant. 

7.30pm Professor Doursounian comes in and sees the bed pan. "She can't use that, take her to the toilet!" he said.

An immediate spectacular success! Thank goodness he came to see me! 

On the first night I got up with help as I had oxygen, the two drips, and the drain. Not too much difficulty amd hardly any pain at all.

Anaesthetist

J'ai vu l'anesthésiste ce matin sur Paris. Elle m'a fait une ordonnance pour des bas de contention pour après l'intervention. Je pèse 60kg et mesure 1m52, donc deux ou trois kilos en trop. Elle pense que je vais être immobilisée un moment et je lui avais dit que j'avais fait une phlébite à 18ans. J'en ai 61 maintenant! J'aurai du garder ça pour moi-même, je pense. Également régime sans résidu jusqu'à lundi, c'est à dire pas de fruits ni de légumes, pas de pain ni de viande rouge. Que du jambon blanc, des pâtes ou le blanc de poulet et beaucoup d'eau. Elle vise l'intestin quasiment vidé lundi. Je ne vois pas comment. Aussi je serai à jeun à partir de dimanche à 17h. L'opération est prévue lundi matin. J'aurai une piqûre pour me détendre à 7h du matin. On vous donne l'anesthésie générale au bloc, puis on vous retourne pour vous mettre sur le billard après avoir été endormi. Que je suis contente que c'est comme ça. Je n'aurai pas le wi-fi à l'hôpital mais je vais prendre des notes pour publier une fois que je l'aurai, de retour chez moi. 

La bonne nouvelle c'est que mon fils qui est ambulancier va pouvoir venir me chercher pour me ramener dans le Gers allongée. J'ai de la chance car c'est pris en charge avec ma mutuelle. Le voyage  dure 7 heures mais au moins je serai en famille. J'espère qu'ils me garderont jusqu'à lundi 25 mais il y avait question de me faire sortir le 22...jour de congé pour mon fils. Je vais devoir être ferme et les supplier de me garder jusqu'à lundi.

I saw the anaesthetist this morning in Paris and she has prescribed support stockings for me for after the operation. I weigh 9st something (60kg) and am 5'1", 1m52, so not too overweight, a few pounds that's all. She is thinking of me not moving for a while and I told her I had phlebitis at age 18, I am now 61! I think I might have been better keeping that to myself. Also diet till Monday of no vegetables or fruit, no bread or red meat, just boiled ham, pasta and chicken the white meat only and lots of water. It's called a diet without residual food. She's aiming on my bowel being pretty empty by Monday somehow or other. Don't really follow that. I will be without food from Sunday at 5pm. Operation Monday morning, pre-med at 7am. They give you the general anaesthetic at the operating theatre and then turn you over once you are out. I am glad about that. I kept wondering about it but didn't dare ask you on here. I won't have wifi in the hopital but will take notes and write up once I can post. Good news is that my son, who is a paramedic, will be able to come up to Paris in the firm's ambulance and take me back lying down in it. I consider myself very lucky as it is covered by my insurance too. The trip is 7 hours but at least I will be with family. I am hoping they keep me the whole week till Monday 25th, but there was talk of Friday 22nd...when my son is not working. I will maybe have to assert myself and beg for Monday. 

Off to Paris

Demain matin je prends l'avion pour Paris. Je vois l'anesthésiste vendredi matin  et rentre à l'hôpital  dimanche soir. Opération lundi. Je ne sais pas si j'aurai Internet mais je vais prendre des notes et vous aurez l'opération pas à pas, puis mes réactions.

Tomorrow morning I fly up to Paris. I am seeing the anaesthetist on Friday morning and go into hospital on Sunday evening. Operation on Monday. I don't know if I will have Internet but I am going to take notes and you will get the operation step by step, then my reactions.

I have just joined the Coccyx Pain Support Group on Facebook. Can't work out how many members there are but it is a busy page! It does help to know other people are reading you and offering support. I will certainly do that after this operation. There is only one specialist in France as I said in another post. Flying up to Paris on Thursday 14th. Seeing anaesthetist on Friday 15th.

Chaise

La chaise est arrivée, jour de mon anniversaire en plus! Facile à mettre en place, avec housse, mais je ne pense pas la transporter beaucoup... On peut lire aisément dessus, le "plateau" devant se met plat ou incliné donc on pourrait aussi manger, dans des assiettes plutôt rectagulaires car elle n'est pas très large. Le coussin pour la tête pose problème si vous portez des lunettes mais pour lire vous n'avez pas besoin de ce coussin. Il s'enlève facilement comme toutes les parties. Ce coussin sera utile pour se reposer ou se faire masser le dos.

The chair came today, and it's my birthday too! Easy to assemble, with a holdall, but I don't think I will be moving it around much...you can read easily on it as the "tray" in front can be inclined or flat so you can eat on it too. I'd say on rectangular plates more likely as it is not that wide. The cushion for your face poses a problem if you wear glasses but when you are reading you don't need it attached. It detaches and adjusts easily as do all the parts. The face cushion will be useful to rest on or to have a back massage.

Arnica

J'utilise l'arnica depuis toujours. Mon père utilisait la pommade pour attenuer la douleur des contusions. J'avais pensé commencer une cure dès lundi. Cela serait 15 jours avant l'opération. Je le prendrai sous forme de granules. 

Cependant, je viens de trouver des indications pour la prise. 

"dans les soins pré et post-opératoires on utilise une dose d'Arnica montana 9 CH par jour pendant 3 jours qui précèdent l'intervention.

Puis, après l'intervention, on prend 5 granules d'Arnica montana 9 CH toutes les heures dans la 1ère demi-journée, et on espace les prises suivant l'amélioration sur une semaine." 

Je vais faire comme l'on preconise.

I have always used arnica. My father used it for lessening the pain of bruising. I had thought of starting a course of it on Monday. That will be a fornight before the operation. I'll take it in granule form.

But I have just found the directions for taking it.

"In pre and post-op care take one dose of Arnica montana 9CH per day for 3 days before the operation. 

Then after the operation take 5 granules of Arnica montana 9CH every hour on the first half day then space out the doses as improvement takes place over a week."

I'll do what they say.
